To prevent falling through the access opening at arrival area from an access platform the opening must be provided with a self-closing gate. For machinery access - self-closing gates must meet the requirements of ISO 14122-3:2016, 7.4.1 and the gap between the platform and bottom of the gate does not exceed 500mm. All other ladders - refer to SLLS 200910 - Fall prevention for roof ladders

6/1/2026: (okaforug) Team Assigned to has been changed from Amazon Ops Eng to TGW so the status is being reset to Pending 5/25/2026: (anmgnz) Despite the fixed ladder has self-closing gate to prevent falling from the top of the ladder, the doors are not closing. Observed in different locations on the site. ISO 14122-4:2016 clause 5.4.4.2. A self-closing gate should be installed at the access point on the platform to reduce the risk of falling through the opening. "Self-closing gates shall meet the requirements of ISO 14122-3:2016, 7.4.1."
